There are some pretty easy-to-follow steps on the NJ.gov website dedicated to the Division of Medical Marijuana. 

How to get a medical marijuana card in New Jersey 

Below is a breakdown of the full process and the necessary steps. 

First, there are certain qualifying conditions for a medical marijuana card in New Jersey. Here’s the list provided on the government website.

  • Amyotrophic lateral sclerosis
  • Anxiety
  • Cancer
  • Chronic Pain
  • Dysmenorrhea
  • Glaucoma
  • Inflammatory bowel disease, including Crohn’s disease
  • Intractable skeletal spasticity
  • Migraine
  • Multiple sclerosis
  • Muscular dystrophy
  • Opioid Use Disorder
  • Positive status for Human Immunodeficiency Virus (HIV) and Acquired Deficiency Syndrome (AIDS)
  • Post-Traumatic Stress Disorder (PTSD)
  • Seizure disorder, including epilepsy
  • Terminal illness with prognosis of less than 12 months to live
  • Tourette Syndrome
  • The MMP (Medical Marijuana Program) card fee is $100. If you are 65 or older, a veteran, receiving disability benefits, or a recipient of other state benefits then you could qualify for the reduced fee of only $20. 

There are specific documents that you’ll need to properly complete the application process. 

Here’s what you’ll need to apply for your medical marijuana card:

  • Photo of yourself (Taken within the last 60 days against a plain white background, no hats, glasses, filters, or smiles. Straight on, full face)
  • Government Issued Photo ID – NJ Driver’s license, NJ non-driver ID, or County ID (current, valid, and must reflect current address)
  • Proof that you’re a New Jersey Resident (from within the last 90 days)- utility bill, IRS letter or NJ tax letter, letter from Federal-State-Local government, or a bank statement

First you need to go to the Registry Homepage and complete the New Patient Registration

Here are the steps in the application process as provided on the NJ.gov page:

Step One: Go to https://njmmp.nj.gov/ and open Patient Registration page.

Step Two: Enter all required information on Attending Health Care Practitioner’s Statement.

Step Three: Complete required patient information (as noted by the red asterisk).

Step Four: Enter the Alternative Treatment Center of choice. This can be changed at any time.

Step Five: Complete patient certification.

Step Six: Complete required caregiver information (if applicable).

Step Seven: Click “Save and Continue” and review the confirmation page.

Step Eight: Continue to upload documents for both patient and caregiver (if applicable)

According to the NJ.gov page, “The MMP will review your application and supporting documents. All applications will be responded to via e-mail with further instructions to finalize your application. Approved applicants will be emailed instructions on the MMP e-payment process to pay the card fee.

Once notified by the MMP of the fee amount, the patient will be prompted to return to the registry homepage and click on payment (you will need your patient reference number). Click submit and complete the required information (as noted by the red asterisk). If submitted correctly, you will receive a transaction confirmation number on the last page.”
